Jane is a 73 year old female.  She lives alone in the home where she raised her family in an upper Midwest US State.  She is a retired grade school teacher and has adult children living nearby with kids of their own.  She loves reading, sewing and baking sweets.



Jane's Values and Fears

Jane recently learned that she has Type II Diabetes and High Blood Pressure and is concerned that it will mean that she can’t enjoy life anymore.

Jane loves spending time with her kids and grandkids and wants to enjoy her retirement and not be controlled by her conditions.   

The Value of Telanurse

Telanurse provides Jane with hope from having a Care Partner that is monitoring her glucose and blood pressure.  She receives text reminders and praise for meeting goals.  She sees that with her partner in health, she can stay on top of her conditions all from the comfort of home and continue to enjoy life.

Receives initial direct mail from Telanurse.

Visits Telanurse website to learn about telehealth.
Phase Two

Engage and Respond

4.

Receives Call from Telanurse Care Coach and completes Health Risk Assessment (HRA) over the Phone.

5.

Hears about Remote Patient Monitoring, but is skeptical.

6.

Visits Telanurse website to learn about Remote Patient Monitoring.
Phase Three

Build Care Plan

7.

Joins the scheduled Annual Wellness Visit from her computer.

8.

Goes through the results of the Health Risk Assessment and finds out that she is a good candidate for Chronic Care Management and Remote Patient Monitoring.

9.

Wants to talk to her Primary Care Physician.
Phase Four

Partners in Health

10.

Primary Care Physician reviews her HRA and provides support for next steps.

11.

Goes to the Telanurse Patient Portal to set up next appointments.

12.

Receives devices to monitor Health. 

13.

Checks in monthly with her care coach.

14.

Sees Symptoms Improvement.
