What is the difference between the Annual Wellness Visit and my yearly physical?
The Annual Wellness Visit gives your healthcare provider a better idea of your overall current health and identify possible risks, so that a preventive health plan can be developed to keep you as healthy over time. Your yearly physical is more extensive, involving a complete head-to-toe, in-person assessment by your physician. While the Annual Wellness Visit is usually covered by Medicare, a yearly physical is not covered by Medicare Part B.
Are Telanurse visits covered by my insurance?
Medicare Part B covers the preventive health services we offer if you meet the eligibility requirements and guidelines. Each preventive service has its own eligibility requirements, and the coverage rules for different Medicare plans may vary, so we recommend checking with your health plan to confirm your eligibility and to find out if there will be any out-of-pocket costs, such as copays.
How do I schedule a visit?
To schedule a visit, log in to your OnPatient account at telanurse.net and click the “Schedule an Appointment” button….?? You can also call Monday through Friday between 8:00AM and 8:00PM Eastern time for assistance.
Am I still able to participate without a computer or internet?
Yes, we can help you complete your appointments over the phone. However, we will work together to find a way to meet over the internet by video so that we can talk face-to-face and perform visual assessments as part of the Annual Wellness Visit.
Does Telanurse work directly for the practice, or independently?
Telanurse partners with medical practices, operating as an extension, to enhance patients’ access to care and guide their journey to wellness. We are not employed by the practice.
How do I sign-up to be a member?
To become a member, all you need to do is complete your Health Risk Assessment. Here’s how:
•Visit the sign-up page [link “sign-up page” to the HRA request form] to request your annual Health Risk Assessment (HRA).
•Click the secure link in your confirmation email and set a password to create your profile.
•Complete your HRA online by answering a series of multiple-choice questions. It’s easy and should take less than 30 minutes.
If you are not comfortable completing the HRA on your own, you can call Telanurse at Monday through Friday between 8:00 am – 8:00 pm Eastern time for assistance.
Can I do the HRA via mail if I am not comfortable sharing my information over the phone or online?
Yes, although we prefer that you complete the HRA securely with one of our coaches or online, we can send you a hard-copy HRA to complete and mail back to us with a prepaid envelope.
Does Telanurse share my Health Risk Assessment and other health information with my other doctors/specialists?
The information you share with us is confidential. We only share it, with your consent, with your primary care provider and any other healthcare providers you specifically identify.
How does Telanurse protect my privacy?
Telanurse complies with all state and national privacy laws and regulations. We will not share your medical information without your consent, unless required by law. We may use non-identifying information to help us improve our services.
What are the qualifications of the Telanurse care coaches?
Every Telanurse care coach is a Licensed Practical Nurse (LPN) or Certified Nursing Assistant (CNA) who is specially trained to coordinate with medical teams to ensure their patients get the care they need.
Will I talk to my doctor during my Telanurse appointments?
Your Telanurse appointments are one-on-one visits with your care coach that take place in a telehealth setting. Your care coach consults with your doctor on an ongoing basis and may recommend follow-up visits with your physician.
