Telanurse works with you and your healthcare provider to coordinate and support important Medicare preventive care services through telehealth. Through our innovative virtual platform, patients with chronic illnesses such as diabetes, asthma and hypertension, are now able to receive care from the comfort and convenience of their homes.
Helping patients meet their healthcare goals is at the heart of what we do. Every Telanurse Healthy from Home member is assigned a personal Care Coach to provide them with a partner in accessing and benefiting from our program. Our Care Coaches are specially trained Care Coordination Nurses that are experienced in working virtually one-on-one with patients and are passionate and committed to helping you be as healthy as you can be.
Upon completion of the Health Risk Assessment (HRA), you will become a Telanurse Healthy From Home member. You can complete your HRA online in about 30 minutes. If you prefer, a care coach can assists you in complete your HRA. We can also send you a hard-copy for you to complete through the mail.
As a Healthy from Home Member, you’ll have access to the members-only benefits. As part of your benefits package, you will have a Personal Care Coach assigned to be your partner in meeting your health goals in the program. Your care coach will work with your physician to develop a Personalized Care Plan that will provide you with goals for managing your pre-existing conditions and for adopting healthy habits that will improve your overall health and increase your chances of preventing new or worsening conditions.
All Healthy from Home Members have the Annual Wellness Visit as part of their package of Services. The Annual Wellness Visit provides you about an hour of one-on-one time with your care coach to walk through the results of your Health Risk Assessment, go through some other visual and risk assessments, walk through your Personalized Care Plan and more. This is an extremely valuable service that is completely covered by Medicare and guaranteed zero cost to you.
During the Annual Wellness Visit, you may also do Advance Care Planning with your Coach. Advance Care Planning is designed to give you piece of mind. You’ll know that your wishes will be known and communicated with your health care providers. Then, depending on your eligibility, your Coach may recommend one or more of the other valuable services we offer – all coordinated by your Coach. Among those services, you may be able to enroll in and benefit from Principal Care Management (PCM), Chronic Care Management(CCM) or Remote Patient Monitoring. These services are geared toward giving you and your coach the ability to set goals around managing existing conditions and mitigating the risks from new or worsening conditions.
